How did Epstein get removed from suicide watch?

Epstein was placed on suicide watch after an apparent self-harm incident on July 23, 2019 — approximately two weeks after his arrest. He was taken off suicide watch roughly six days later. This decision, and subsequent events, would become central to the controversy surrounding his death.

The July 23 Incident: Epstein was found in his cell at MCC New York in a semi-conscious state with marks on his neck. The Bureau of Prisons characterized it as a possible suicide attempt. His cellmate, former police officer Nicholas Tartaglione (who was awaiting trial on murder charges), reportedly told investigators he had not seen what happened.

Suicide Watch Protocol: Under BOP policy, prisoners on suicide watch are placed in a special observation unit, checked constantly, have all potentially harmful items removed, and typically receive mental health assessment.

Removal from Suicide Watch: Approximately six days after placement on suicide watch, MCC psychological staff cleared Epstein to be moved to a “psychological observation” status — a lower level of monitoring. Within about four weeks, he was in a regular cell in a special housing unit.

The Mental Health Assessment: The BOP’s decision to remove Epstein from suicide watch was based on evaluations by prison psychologists. These assessments concluded he was no longer an imminent suicide risk. Critics have questioned whether these assessments were conducted properly.

What Followed: After being removed from suicide watch, Epstein’s regular cellmate was moved out of his cell for reasons that have not been fully explained. This left Epstein alone. On the night of August 9–10, required checks were allegedly not performed. He was found unresponsive at approximately 6:30 AM on August 10 and was pronounced dead at a local hospital.

Investigation: The DOJ’s Office of Inspector General launched an investigation into the circumstances of Epstein’s death, but as of 2025, no final public report had been issued.
